Alright, I have something. Basically I'm going to need to take down your basic personal stats: height, weight, age, dietary restrictions, parentage, allergies, I'll probably do a dental scan, and I'll definately need to do both an ocular and aural exam, maybe a chest x-ray for the records with your consent, a blood test wouldn't be a bad idea...
