Someone mentioned SAS earlier, saying it was the harder version of going comando 
Elephant trunk? Can't say I've ever seen them... but definately not* the sort of thing I'd go for

OK, SAS, the explanation...
Commandos are specialist soldiers. They undertake dangerous missions that ordinary soldiers do not. To be a commando, you've got to be, in the common parlance, "hard".
The SAS are even tougher than commandos. You cannot join the SAS, you have to be picked. They are even "harder" than commandos.
Now, in terms of underwear, we know what "going commando" means. Well, "going SAS" is similar, only...er...harder...
