There's also a mention that it is confused with the American One-Finger Salute, if they are not similar insults how are they different? I had always assumed they were similar insults, if slightly different in meaning. I was also told that it means 'up yours', is this true? And is it acceptable in familiar company?

The normal translation for the one-finger salute is 'up yours' for obvious reasons. The two-finger salute is normally said to mean 'f*** off'. Whether these two phrases actually mean the same thing is open to debate.
