They used cunning camera trickery, doubles and already recorded sounds. Like when he says "Shadows and dust" before he gets killed, he said that earlier when putting Maximus in the arena.

We said that we’d keep you up to date with news about the movie and we have just heard that after all these years, the movie finally has a green light. More news as we get it, but those on the inside say its looking good. The Adams’ Estate is also fully supportive, which is also good news.
