It's on the front page,if I remember rightly it's the first 2 letters of your first name first 3 letters of your surname that makes up your jedi forname and the first 2 letters of your mothers maiden name and the first 3 letters of the place you were born makes up your jedi surname
That's how I ended up with Cobal Th'chi
