I think the authors tried to say it wasn't meant to be a swear word, cos every kid in the country was calling each other 'smeg heads'. But it's too much of a coincidence not to come from 'smegma' and be used to describe Mr Rimmer (Not our Capt'n Rimmer, the one from the show)
