Thinking a bit more about this, for my parents wedding anniversary *years* ago, I managed to get hold of an old photo of their wedding from the local paper back then (which they didn't have) and got that printed for them.

As it's her birthday, perhaps there's a particularly endearing picture of her as a baby or small child which would suit.
You can print the motive onto the transfer paper, then cut out round it, and just iron the part of the picture you want on to the canvas. You could even paint in the spaces round the outside, with a colour to match the picture, or sky blue, using a sponge technique.
As you've already got the hotel and so on sorted out, but a spa weekend would be too expensive, how about a fluffy and a small bottle of massage oil and give her that back rub in style!
Or just wash her hair. There aren't many women who didn't swoon over that scene in Out of Africa where Robert Redford washed her hair.
You have to be able to do it, though. I couldn't think of anything worse than have my husband wash my hair. Personally, I'd prefer a voucher for a session at the hairdresser's!
Have a nice time. You still haven't said exactly when it is ....

The occasion is the day after tomorrow. The only aspect of the 'period' suite is a large, round'ish massage bath that could (in theory) fit 3 or 4 adults. And some scented beads and oils are provided, as I recall from a few years ago. The other time we used that same suite. But thank you for the ideas.

Just as a follow-up ... By accident (literally, but someone else's) she found herself with the day off and spent leisurely. Dinner with a good handful of friends joining us surprised and pleased her. And the best/nicest suite at the B&B was just the topper. She had a fabulous and memorable day.

She really did, the placemat for her restaurant dinner (drawn by Wowbagger) was a big hit. The B&B have asked to use a couple of segments of his handiwork for their own adverts.
