Lieutenant is a commissioned officer.
Segeant-Major is non-commissioned.
BTW
If Lieutenant was a VC, then he would outrank full Major. VC MUST be saluted before salute is returned!!

GT the sgt. major reference will become clear in the expination
it just nudges it the right direction, also a sgt. major is not a non commisioned officer he is a warrant officer and hold the queens warrant

The rank
Major General is in fact Sergent Major Genreal therefore he is outranked by a Lutenant general.

It was a result of the reorganisation og the army by Cromwell as the
the original order was.
Captain General.
Lieutenant General.
Sergeant Major General.
Sergeant Major General was shortened to Major General and by the 18th century the shortened title was the norm.

when the new model army came into existance the captain general was in charge, the second in command was the lieutenant general and in command of the cavalrey, next came the sgt. major general and was in command of the infantary,
over time the captain and sgt. were dropped from the generals titles untill we came to the ranks, in accending order, of

And of course a `Drum Major`, the guy with the big stick who leads a marching band, will outrank all other ranks among the musicians while in band formation. Oh yea, some of them piccolo players are Captains, some of them trombonists are Lieutenants.
