I can't figure out how to start a completely new conversation.

You just have done! 
Each Guide Entry has a 'Start a new conversation' link at the bottom. Clicking this will bring up a text box. When you press 'Post Message', your message will be added to the Entry as a new conversation.

Instead of 'Start a new conversation' you may also find
'discuss this entry', giving the same result.

Guide "Entries" include help pages and people's personal spaces. The wording of the link text may vary a bit depending on the type of page and on the skin you are using. "Leave a message" is another version.
