When I have called a man sweet in the past it has always been in the 'you are wonderful, you did a nice thing, if only I fancied you I'd marry you right now' kind of way.
In other words, I didn't fancy the man.
That doesn't mean I didn't come to fancy him later when the scales fell off of my eyes and I realised that he was in fact quite sexy, and it also doesn't mean he wasn't a jerk (just good at hiding it at first).

But of course - we have different understandings of many/most words, and their dictionary meaning only gives the vaguest idea of what a person "means" by it at any moment in time (see the British English sequel thread for discussions on word meanings).
Anyone remember the usage "she's sweet on him" - that DID mean intentions to do the bed & baby things.
One has to interpret the spoken word in the contexts of who said it, how it was said, when it was said and to whom it was said ... i.e. like the rest of love and life, this isn't an exact science !
