To me that means somebody that has had the pycological evaluation, lived 'as' a woman then the surgery, take he pils and made the change.

I think age has alot to do with it, sexuality. People 'finding' themself. How man lads in their 20's can't take a joke that hints at them being gay. Calling them 'Puff' may be meant as a 'girly bloke' but they get in a tis over it.
