I thought this was very well written and researched. One question I still have after reading it is this: you say that not all hospital have the correct equipment and expertise... do you think it would be worth finding out where some centres of excellence exist in the UK, Europe and USA, perhaps - so you don't end up with a huge list- or maybe find a link to a list if you can....
