Ah, but Pratchett has included him retroactively.
Reg Shoe is in the book, as you know. From the description in the book it is clearly him wearing the sash and waving the flag on the front cover.

You've seen what Slant looks like in Last Hero, Kidby has a distinct zombie style.

Yeah, but in the 'historical' part of Night Watch Reg is still human. He dies at the end.
So the cover, which shows characters from that period, shows Reg as alive. Because, at the time, he was.
