I just read about this on boingboing.net. Apparently one Waffle House or perhaps the whole chain, uses a code of placing condiment packets on plates to indicate what the order is. The kind of condiment and its position on the plate indicates what the cook needs to fill the plate with. According to the caption on this flickr stream, the waitress shouts an order to the grill cook and the grill cook sets condiments on plates in a certain way, basically writing the order in code. That's how they can keep up with several orders at once without having to remember them all.

SO...how many of what sort of condiments are placed on the plate to indicate the addition of cockroaches to one's order?
