He died in 1617. The hospital was founded in 1618, and he lived in it? Did he come back from the dead? That would be a miracle suitable for a saint

Are you confused because you don't know what 'made provision for' means? It means 'left money in his will.' Or that 'here' means 'where the sign is,' which I don't suppose is the wall of the hospital?

"Thomas Anguish 1538-1617. Mayor of Norwich in 1611, his will made provision for a children's hospital, founded in 1618, lived here."
It implies that he may have lived in the hospital, the hospital being "here.".

It wouldn't if you were standing in front of the plaque, I think. You would see that the plaque was not on the wall of a hospital.
He couldn't have lived in a building that wasn't there yet. Unless we're thinking in metaphorical terms, as in Hebrews 11:10.
