Could be both. (I know those adjustable date stamps. If you set it incorrectly, just stamp again with the right settings).

Very possible. I know that era: lots of mistakes were made like that because several million people were trying to manage a totally unfamiliar and suddenly created bureaucracy. There were a lot of jokes about how weird it all was.
