I dispute drive through rule 3. The drive-through is an LA invention.
People in LA use the drive through, because nobody walks here, if they can at all help it. It is a very strong cultural meme a tourist here will need to know. I see 10 car long lines in drive throughs with no-one parked outside. I can walk in, order my food to go, and leave before three cars have left the queue-because nobody is inside.
The other reason to do drive through in LA is privacy; you get the benefit of a meal you haven't cooked, but can consume it either in your car as you are going somewhere, or at home. Also, no one in this land of health-nuts will be likely to see you eating badly if you stay in your car.
The speed expectation is management generated: it's cheaper to feed folks who aren't using your dining room, you don't needs as big a dining room, to hire more people to clean it, or deal with the garbage generated as your customers take it away.
