Does anyone know if it is safe to eat the green stalk which grows out of the centre of an old onion when it's been left in the dark for a while - it's quite like a spring onion to look at.

Perfectly safe. No worries. That's just the shoot of the new plant.

Yes. The flower stalk is safe to eat, cooks up rather leeks in soups and stews but must be harvested while its still in the tender or meristematic phase (susceptible to snapping with finger pressure). Also great in salads.
