I've worked from home in the past and all this is good advice, particularly taking breaks and getting out.
One other bit of advice that worked was to set up a virtual office, which is basically a business address and can also include call answering. It adds that bit of professionalism and I found more people were happy to work with me after I changed the address from a normal home address.
Bear in mind Street View these days too as people can look directly at the front of your house as you use it as your business address!
