Here in Florida, the prefered method is with the help of SCUBA and a sturdy pair of fins. Use of an exit line is mandatory, as is proper training.

Underwater caving hasn't really appealed to me, though several of my mates are (or at least were) quite seriously into it.
I *might* end up doing a through-trip in a month or two that involves a couple of sucks on a bottle to get through some short sumps (12ft and 27ft) that are wide and clear and generally free-dived, but I doubt I'll progress beyond that level.
