This is same article I copied and did some work to it. You'll probably find it easier to print out both and look at them side by side to see the differences. I haven't done a total re-write for obvious reasons but hopefully I've done enough for you to be able to see where things aren't right and why.

There's so much more you could do towards the end of this to flesh out the info and thus preventing it looking both like a list and rough notes.
Instead of a sub-header of 'Notes' use something like 'Additional Information'.
