I have the Grove Press paperback copyright 1965. It's a plain white cover with a simple box on the front with the title and author. there are also some quotes front and back from Eliot Fremont-Smith, the NY Times reviewer:
"A total, authentic literary experience ... the free publication of 'Story of O' in this country is an event of considerable importance ... it is a significant measure of how far we have come in lifting the restrictions on art and our responses to it ..."
There are some more reviews inside, but oddly none from the right wing ...
