Perhaps you pressed the "Add Journal Entry" button twice without realising it? Unfortunately browsers aren't very good at protecting against that sort of thing happening accidentally.

This can happen if a connection is dropped after the server has accepted a form submission but before it returns the results to the client. The user thinks, "shit", hits Back to return to the form and re-submits.

If you lose a connection while posting a form, return to the form, copy any important data (journal entry text) and then Refresh the page your wanted to update (your journal). If the server heard you the first time, you'll see your entry - if not you're safe to post again.

Yeah, I've had this problem with adding user pages. I type it in, add it, find I want to change something then click Back and accidentally re add it. A way to delete entries would be good.
