2) you can change this by going to doubleclick.net and using the privacy options. 
4) Practically, we do indeed have the choice to watch ads or not. sites such as the one you mentioned do work.
I would be vaguely happy to see a "premium" option, that allowed people to pay a monthly fee in return for no adverts (and scrapping that irritating WAP ad too!!). That way, rich people who hate ads can get rid of them, while poor people who would be unable to access h2g2 if they had to pay for it, would be able to access it.
