I don't know where the INDEX comes from, but I always remove them when I come across them. The footnotes should not be numbered, because if you add an extra one before the last one, they'll all have the wrong numbers.
The Shorter Oxford English Dictionary gives meaning 5a of swim (noun) as "A deep pool in a river etc. frequented by fish."
