The "accident report" is actually from a talk given by Gerard Hoffnung (of musical cartoon fame) in 1958, to the Oxford Union:

Also very amusing is the "French Widows" skit on the flip side of the same (rare) 7" 33rpm record, which my Dad has a copy of.
