It is not physically possible to open a plane door on a commercial flight at high altitude. You have to pull the door in to the pressurised cabin before you can swing it open but the pressure difference between near vacuum outside and a lowish one atmosphere inside sticks the door fast.
However, not everyone leaving a plane at altitude leaves through a door - if the rear half of the plane is missing (or the front half if you have cheaper tickets), you may be able to leave through the gaping hole in the fuselage bypassing the door problem altogether.
In this case, don't forget your life jacket, light and dinky whistle. If you survive the fall, you will kick yourself if the search plane misses you because you didn't have a whistle.
