When Codd coined the phrase in 1993, did he perhaps hyphenate "on-line"? My dictionary's only 5 years old and it has "on-line" hyphenated. And Codd must have been getting on a bit even 7 years ago.

Hmmm, you may be on to something there. I don't happen to have a copy of Codd handy at the moment, bu that other datatbase standard Elmasri & Navathe consistently hyphenates "on-line" and "off-line".
