In an instrant runoff, voters rank candidates in order of preference on their ballot. The runoff occurs mathematically as part of vote tallying, which is why its instant as opposed to sequential. At least that's how I understand it. The Center for Voting and Democracy has a good description of it here: http://www.fairvote.org/irv/whatis2.htm.
