"There is no country in the world where obesity is not rising." Vatican maybe? 
I read somewhere about a Mexican guy who weighs (or used to weigh, it was last year) about 550 kilos, and was unable to move at all for several years. Eer, shouldn't he have worried when the first digit on his scale became "2" already? And decided to take dramatic action when it turned to "3"? You don't just wake up one morning and you're 500 kilos, do you? 
Why could it be that this person took so long to try and do something about his condition?
