this has been useful as i am suffering from a life threatening illness and had never heard or been told about the chronically sick or disabled registration form.My line has been down a few times and i've had to go longer than i'd have liked to go without my lifeline the telephone .

got the form and have filled it in and sent it back,thank you this should make a difference to getting repairs done a little quicker,if they accept it.
