The band "Hawkwind" released an album called "PXR5" (I was listening to it earlier! ) whose cover design consisted of an oversized diagram of a wrongly wired plug. They were all recalled from the record shops, allegedly after a really stupid person used one as a reference and blew out the main fuse in his house.
They reappeared a few days later with a large adhesive label covering the "wiring", cheerfully announcing "Please do not remove this sticker".
Oddly, although the record company issued a reference code for the CD version, I have yet to track down anyone who ever saw a copy on CD (other than bootlegs) - I wonder if there is a connection? (pun unintentional )
