Oh, and from what I remember of my nephew's first birthday...
Have a big box wrapped in lots of paper, containing balloons - that's what my nephew seemed to be most interested in, rather than what the gifts themselves...

She had a great birthday
Only part she didn't like was getting her hands all clogged with icing
Probably didn't help that the person she was expecting to rescue her laughed like everyone else and took pictures I don't think she expected the toys to still be here when she got up--I brought her into the living room this morning and her face lit up hugely!
