Interestly, there is a tombstone on the engineering quad of Cornell University in Ithaca, N.Y. which mourns the passing of the Dutch Elms that once lined the campus's main route.
The university has a highly regarded agricultural college, which may offer some explaination for its tree-related grief.
Unfortuately, I am unaware of any other tombstones for elm trees in the U.S. or elsewhere.

John the gardener says, "Free Tibet!"

The trees were probably American Elms. The disease is "Dutch".

There ought to be a significant monument in every settlement in the Northeast. The loss of elms there (Canada and the United States) is nothing short of a tragedy.
