This is pratically impossible. The only way to take enough air into your lungs to run the risk of exploding is to form a gas tight seal with your lips. With the 3000 psi inside a SCUBA cylinder, that could prove a bit difficult.

It is possible to breath straight from the pillar valve, dispensing with the regulator and demand valve altogether. The only problem with this is that most of the available air is wasted. You would empty an 80 cft cylinder in a matter of minutes.
